• Please fill out and submit the registration form below.
• If you have more than four children, please submit an additional form.
• The release of liability form can be found at the bottom of this page.  Please print it, fill it out, and bring it with you the first night of VBS.

• The document below contains both the registration form and release of liability form.
• If you already submitted your online registration form, you only need to print and fill out the release of liability form. Please bring it with you on the first night of VBS.
• If you are unable to fill out the forms ahead of time, you will be asked to fill them out on site.
